HR/Total Rewards Team completing audit, verifying dependents for benefit coverage
The Total Rewards Team within Human Resources is completing an audit for Dependent Verification for Benefit Coverages. An email is being sent to employees who have dependents on their benefit coverages, but are missing the verification documents in their file. It is part of our plan policy that this document is on file in order to continue with coverage. This is an audit that has been missed in previous years, but will continue to be an annual audit moving forward. Only employees who are missing the documents will be emailed; others who have dependents, and whose documentation we already have, will not receive an email.

Documents may be uploaded through the ICUBA Benefits portal. Login to ICUBA through Okta, then go to My Profile on the left side of the page. Click on Edit My Profile. Click on Employee File. You will see Upload Documents. On the right, you will see Add Birth Certificate, Add Marriage Certificate, and other documentation. Click on that and add your files.
