Human Resources partners with Walgreens to offer flu shots
Have you received your flu shot yet this season? If you haven’t, Walgreen’s will be hosting a flu shot clinic from 11 a.m. to 2 p.m., on Wednesday, November 10, in the Greenfelder-Denlinger Boardrooms A and B at University Campus. Be sure to bring your ICUBA Optum RX ID cards with you.
The flu shot clinic is first come, first served. Please plan for a 15-minute wait before receiving the flu shot. To save time, please bring a copy of the completed Informed Consent for Vaccination form, as well as a copy of your insurance ID card attached to your completed paperwork.
